DIRECT BANKING DATA ENTRY FORM
Each applicant must fill out a separate Data Entry Form in block letters.
The fields that each applicant is required to fill out are as follows – NAME, ADDRESS. The applicants must also answer three of the security questions listed on the form.
The Applicant is required to sign on the line marked "Customer's Signature"
The other fields will be filled in by a Banking officer.
DIRECT BANKING AGREEMENT
For Company OR Individual - only one Agreement is needed as appropriate.
The names of all applicants must be stated under Account Name as well as all the relevant information on the front of the form.
When you have read the terms in the agreement, each applicant must sign under Customer’s Signature on the last page of the Agreement.
When you have completed these forms, if you are a local customer, you would be required to take same into your GBTI Branch to be signed up for the service.
If you are an overseas customer, kindly send same along with a written correspondence requesting that you be signed on with this service. The letter must be addressed to your Branch Manager. The letter, as well as the forms, must be authenticated by the Guyana Consulate or your Banker in your country of residence.
